Is a Vegetarian Diet Better for Weight and Fat Loss?


    Obesity is a major global health challenge. According to the World Health Organization (WHO), 2.5 billion adults—43% of the world’s adult population—were overweight in 2022, including more than 890 million living with obesity. Adult obesity has more than doubled since 1990, and approximately one in eight people worldwide now lives with obesity (1).

 

    Excess body fat, particularly visceral fat surrounding the internal organs, increases the risk of type 2 diabetes, hypertension, abnormal blood lipids, cardiovascular disease, fatty liver disease, and certain cancers. WHO estimates that a higher-than-optimal body mass index contributed to approximately five million deaths from noncommunicable diseases in 2019 (2).

 

    Many dietary strategies have been proposed for weight management, including low-carbohydrate, Mediterranean, calorie-restricted, and vegetarian diets. Vegetarian diets exclude meat but may include eggs and dairy products, while vegan diets exclude all animal-derived foods. Although comparisons with nonvegetarian diets have produced mixed results, several clinical trials suggest that well-planned vegetarian diets—particularly low-fat vegan diets—may offer advantages for weight and fat loss.

 

Comparing Five Dietary Patterns

 

    In a six-month trial, 63 adults with overweight or obesity were assigned to a vegan, vegetarian, pesco-vegetarian, semi-vegetarian, or omnivorous diet. All diets were low in fat and had a low glycemic index, but calorie restriction was not required (3).

 

    The vegan group lost 7.5% of initial body weight, compared with 3.1% in the omnivorous group and 3.2% in both the semi-vegetarian and pesco-vegetarian groups. Vegan participants also reduced total and saturated fat intake to a greater extent than these groups. However, the study was small, and only 79% of participants completed the intervention (3).

 

Low-Fat Vegan Diet in Postmenopausal Women

 

    In a 14-week trial, 64 postmenopausal women with overweight followed either a low-fat vegan diet or a conventional cholesterol-lowering diet. Neither calorie restriction nor additional exercise was prescribed (4).

 

    Women following the vegan diet lost an average of 5.8 kg (12.8 lb), compared with 3.8 kg (8.4 lb) in the control group. Insulin sensitivity improved within the vegan group, although the difference between groups was not statistically significant. These results suggest that a low-fat vegan diet may support weight loss even without prescribed calorie or portion limits (4).

 

A Vegan Diet in the Workplace

 

    An 18-week study conducted at 10 U.S. corporate sites included employees with overweight, obesity, or type 2 diabetes. Participants followed either a low-fat vegan diet with weekly group support and workplace meal options or made no dietary changes (5).

 

    Across all participants, the vegan group lost 2.9 kg (6.4 lb), compared with 0.06 kg (0.1 lb) in the control group. Total and LDL cholesterol decreased by 8.0 and 8.1 mg/dL, respectively. Among participants with diabetes, HbA1c decreased by 0.6 percentage point, compared with 0.08 percentage point in the control group (5).

 

    Among those who completed the study, weight loss averaged 4.3 kg (9.5 lb) in the vegan group and 0.08 kg (0.2 lb) in the control group. Total and LDL cholesterol decreased by 13.7 and 13.0 mg/dL, while HbA1c declined by 0.7 percentage point, compared with 0.1 percentage point in the control group (5).

 

A High-Carbohydrate Vegan Diet

 

    In a 16-week trial, 75 adults followed either a high-carbohydrate, low-fat vegan diet or their usual diet. Physical activity remained largely unchanged, and both groups reported similar reductions in calorie intake (6).

 

    Compared with the control group, the vegan group consumed approximately 71 g more carbohydrates, 15.3 g more total fiber, 12.5 g more insoluble fiber, and nearly 50 g more starch per day—without a significant increase in sucrose. Despite the higher carbohydrate intake, the vegan diet produced a 6.5 kg (14.3 lb) greater reduction in body weight and a 4.3 kg (9.5 lb) greater reduction in fat mass. Insulin resistance also improved, with HOMA-IR decreasing by 1.0 point relative to the control group (6).

 

    Greater increases in carbohydrate and fiber intake were associated with larger reductions in BMI, total fat, visceral fat, and insulin resistance. These findings suggest that carbohydrate quality and food source may be more important than carbohydrate quantity alone (6).

 

Vegan Diet and Type 2 Diabetes

 

    In a small 12-week pilot study, 11 adults with type 2 diabetes followed either a low-fat vegan diet or a conventional low-fat diet, without additional exercise recommendations (7).

 

    Fasting glucose decreased by 28% in the vegan group—from 195 to 141 mg/dL—compared with a 12% reduction in the control group, from 179 to 157 mg/dL. Average weight loss was also greater with the vegan diet: 7.2 kg (15.9 lb) versus 3.8 kg (8.4 lb) (7).

 

    Several participants following the vegan diet reduced or discontinued their diabetes medications, while no one in the control group did so. However, HDL cholesterol decreased more in the vegan group, an unfavorable result. Because the study was very small, its findings require confirmation in larger trials (7).

 

Vegetarian Diet Combined with Exercise

 

    In a 24-week trial, 74 adults with type 2 diabetes followed either a vegetarian diet or a conventional diabetic diet. Both diets provided 500 fewer calories per day, and all meals were supplied. Aerobic exercise was added during the second 12 weeks (8).

 

    Compared with the conventional diet, the vegetarian diet produced:

 

·      Greater weight loss: 6.2 kg (13.7 lb) versus 3.2 kg (7.1 lb)

·      A larger increase in insulin sensitivity: 30% versus 20%

·      Greater reductions in visceral and subcutaneous fat

·      More frequent reductions in diabetes medication: 43% versus 5%

 

    The vegetarian group also showed favorable changes in adiponectin, leptin, vitamin C, and antioxidant defenses. Differences between the groups became greater after exercise was added (8).

 

Summary

 

    These studies suggest that well-planned vegetarian and vegan diets—particularly those emphasizing low-fat, fiber-rich whole foods—can be effective options for weight and body-fat loss. Vegan diets produced greater weight loss than several less restrictive diets in some trials, but they are not automatically superior in every situation.

 

    For people comfortable avoiding all animal products, a balanced vegan diet may be a useful weight-loss approach. For others, a vegetarian diet that includes moderate amounts of eggs or low-fat dairy may be easier to follow while offering similar advantages. Whichever pattern is chosen, the greatest benefits are likely to come from emphasizing vegetables, fruits, legumes, whole grains, and minimally processed soy foods while limiting refined carbohydrates, fried foods, sweets, sugary drinks, and excessive added fats.

 

    Long-term adherence is more important than choosing the strictest diet. The best approach is a nutritionally balanced vegetarian or vegan diet that creates a modest calorie deficit, provides adequate protein to preserve muscle, and is sustainable over time. Vegan diets also require a reliable source of vitamin B12 and careful attention to other potentially limited nutrients.
